The episode begins with Kaneki being taken to a hospital, where he meets Dr. Akihiro Tsukiyama, a doctor who is revealed to be a ghoul. Tsukiyama is friendly and helpful, but Kaneki is still wary of his intentions.
The episode ends with Kaneki having a confrontation with Kiyoshi, a ghoul who is hostile towards him. Kaneki uses his newfound ghoul strength to defend himself, marking a turning point in his acceptance of his new identity.
Meanwhile, Nishio, a young ghoul who Kaneki met earlier, is shown to be struggling with his own identity as a half-ghoul. He is mocked and belittled by other ghouls for his half-human appearance.
